Job Description

Team Select Homecare is looking for Licensed Practical Nurses to join our Team Select family. *Our nurses will document and provide skilled nursing care in a 1:1 setting to our patients in their home.*

* Chester county and the surrounding area!

*We work with pediatric and adult patients!* *Assist the patient in learning appropriate self-care techniques and assure that the patient/family is involved in implementing the plan of care*

*Responsibilities*

* Implement the nursing care plan for each patient.
* Initiate preventive and rehabilitative nursing procedures.
* Teach and counsel the patient and family/significant others regarding the nursing care needs and other related problems of the patient at home.
* Maintain and complete records of observations, treatments and care of patient. Help update the patient plan of care.
* Administer medications and treatments as ordered.
* Demonstrate competent performance of technical skills according to established procedures before performing new skills.

Pay: $29.00 - $35.00 per hour
