Job Description

About CleanSlate:

Join us at CleanSlate Centers, where your work transcends typical healthcare by profoundly impacting lives and restoring communities. As a national leader in evidence-based addiction treatment, we are physician-founded and physician-led—with a clear mission to save lives and create pathways to lasting recovery. As a Center Medical Director, you'll balance meaningful patient care with the opportunity to lead clinical teams, mentor providers, and shape outcomes for some of the most vulnerable in our communities.

The Role:

As Center Medical Director (CMD), you’ll serve as both a clinical provider and a physician leader. In this clinical and administrative role, you’ll manage a multidisciplinary care team while maintaining a full patient panel. You’ll collaborate with operations leaders, guide APRNs/PAs, participate in quality initiatives, and help grow our local presence through clinical excellence.

What You’ll Do:

  • Provide medication-assisted treatment (MAT), including initial evaluations, inductions on Buprenorphine, and follow-up care

  • Supervise and mentor APRNs/PAs and support their clinical development

  • Lead weekly interdisciplinary treatment team meetings (IDTs)

  • Collaborate with Center Manager on quality and performance metrics (retention, attrition, model compliance)

  • Evaluate and address complex patient care decisions, including referrals for higher levels of care

  • Participate in provider education, mentoring, and optional academic research

  • Support local referral relationships and community partnerships

  • Manage clinical documentation, co-signatures, and ensure EMR compliance

  • Partner with HR and People Services on provider performance and employee relations matters

  • Maintain active licensure and complete all credentialing requirements for practice

Who We’re Looking For:

  • Board Certified or Board Eligible in a specialty recognized by ABMS or AOA

  • Fellowship training in Addiction Medicine preferred (not required)

  • Background in Family Medicine, Psychiatry, Internal Medicine, OB-GYN, or Emergency Medicine with addiction experience welcomed

  • Active state license and controlled substance registration

  • Proven leadership capabilities and a commitment to team-based care

  • EMR fluency and strong communication and documentation skills

  • A compassionate, servant-leader mindset and dedication to treating substance use disorders

Why CleanSlate?

  • Monday–Friday schedule – no weekends

  • $20,000 Sign-on Bonus

  • Collaborative, mission-driven work environment

  • Generous PTO + paid holidays

  • Full benefits: medical, dental, vision, 401(k) with company match

  • Company-paid life, disability, and malpractice insurance

  • Continuing Education Support: CME allowance + CME PTO for ongoing training and recertification

  • Opportunity to lead and grow in a respected, physician-led organization

Lead care that transforms lives. Apply today and guide your team—and your patients—toward hope and healing at CleanSlate Centers.
